Guardian Community Living

Founded in 2003, Guardian Community Living provides supported living and clinical therapies to more than 400 individuals with mental retardation throughout Tennessee. GCL has become one of the state’s most respected providers of community services. Our history of success has positioned GCL to offer services to individuals with mental retardation in other parts of the country. Working with families, consumers and state officials, GCL is uniquely qualified to help states address their waiting list, deinstitutionalization and service capacity needs.

Our accomplishments include:
  • Being awarded “Quality Tier Status” five consecutive years by the Federal Court Monitor for the Remedial Order, a federal class in Tennessee’s service delivery environment.
  • Earning exemplary grades from the Quality Review Panel (QRP), the oversight entity for the Settlement Agreement, another federal class within Tennessee’s court-ordered service delivery environment.
  • Achieving consistently high marks through state QA surveys during our unrivaled growth.
  • Opening more than 60 supported living homes during a four-year span, encompassing 200 miles.
  • Building extensive experience and a track record of success in transitioning individuals from state developmental centers into community living settings.
  • Completing successful turnarounds of seriously troubled providers in response to numerous urgent requests by the state under emergency circumstances.
  • Developing a strong working partnership with the Direct Support Professional Association of Tennessee, emblematic of our commitment to our work force.
  • Being recruited, based on our reputation, to provide a full range of clinical therapies for a private ICF/MR after it came under federal court scrutiny.

Experience
With years of deinstitutionalization and person-centered planning experience, GCL has worked closely with the state of Tennessee’s transition teams in successfully supporting individuals’ moves from institutions to the community. Without exception, individuals who have moved from state institutions to GCL-managed community homes have experienced substantial improvement in their overall health status.

Supported Living Services
We offer our supported living services in superior residential neighborhoods. Residential staff are selected and assigned after a thorough screening process, and employees receive ongoing training to ensure that they have the tools necessary to provide high-quality and personalized services.

GCL supported living services include:
  • Company vehicles that are selected with the needs of the service recipients in mind supported by a 24-hour roadside assistance program.
  • Customized community- and home-based day services that run the full range of experiences – from sensory and mobility activities all the way to supported employment.
  • Ongoing healthcare oversight and nursing with 24-hour on-call services through our nursing department of registered nurses.

Medical Residential Services
We provide fully accessible medical residential homes in appealing neighborhoods for individuals who require intensive direct nursing and care oversight. We understand that around-the-clock nursing is a serious commitment, so we have an agreement with our healthcare staffing affiliate, Guardian Healthcare Providers, to have 24-hour access to appropriately trained nurses for those occasions when the welfare of an individual dictates the presence and care of a nurse.

Ancillary Clinical Services
In addition to a range of residential, day and health-related supports, we incorporate the highest quality ancillary skilled therapies and services under one roof. GCL seamlessly provides, manages and coordinates clinical support services, including occupational therapy, physical therapy, speech language pathology and nutrition with residential, day and nursing services.

Vocational Rehabilitation
When an individual under our residential care wishes to work, one of our occupational therapists will assess the patient’s skills and needs and match him or her with the most appropriate work environment. This bolsters GCL’s position as a full-service provider.

Management Services
Economic downturns and rising healthcare and fuel costs can present unpleasant choices for providers who are committed to a mission of quality services for individuals with special needs. In recognition of this, GCL has developed a menu of administrative options for providers to allow them to focus on quality-of-life issues and reduce overhead costs, thereby avoiding a compromise in core values. Due to GCL’s administrative expertise and depth, we offer providers cost-effective solutions to weather the storm or to provide long-term administrative support.

Our menu of management services includes:

  • Financial management, budgeting and forecasting, financial performance analysis, and billing
  • Accounts receivables management
  • Proven quality assurance and quality improvement systems
  • Human resources information management
  • Upper- and middle-level management
