JOB DESCRIPTION:Provide services to patients who have impairments, physical and/or developmental disabilities, functional limitations, or changes in physical function and health status resulting from injury, disease or other causes. Actively collaborates with the health care/educational team to meet patient outcomes and system improvement objectives. Demonstrates knowledge of principles of growth and development over life span and possesses the ability to assess data reflective of patient status and interpret appropriate information needed to identify patient requirements relative to his/her age specific needs.
1.Completes Physical therapy assessment using the appropriate testing method and equipment for the patient type, diagnosis and reason for referral.
2. Formulates functional treatment plan to address deficits and patient/family areas of concern.
3.Provides therapeutic interventions for identified objectives/goals
4.Provides transition and discharge planning
5.Provides education/consultation to patient, family, team and community
6.Completes timely documentation
7. Provides supervision of PTA and Rehab Aides by PA act
PROFESSIONAL RESPONSIBILITIES: Maintains Physical Therapy license in good standing in the state of practice and meets all state licensure continuing education requirements in a timely manner. Notifies employer immediately of any threatened or pending action on occupational therapy license.
QUALIFICATIONS REQUIRED: Minimum education required is a Bachelor’s degree in Physical therapy. Must have a valid Physical Therapist license in the state of assignment. One year of clinical experience preferred. Demonstrated ability to communicate verbally and in writing. Current Basic Cardiac Life Support certification. Must be able to travel to patient homes within designed geographic area, proof of valid driver's license and auto insurance
