We seek Transition Specialists, familiar with Behavioral Residential Services, who can provide assistance in assessment, coordination, and facilitation for the benefit of residents in a facility located just north of the Orlando/Winter Park area in Florida. The Transition Specialists are a vital component of the Transition Team. The goal of the Transition Team is to ensure that those being served by the facility receive the best person-centered care in the least restrictive setting possible. The Transition Team is a mission-critical part of a tripod organizational structure to ensure that all transition goals are carried out.
As part of the Transition Team, the Specialist will complete assessments, coordinate with resident’s family/guardians, and other members of the person-centered care teams, and conduct follow-up activities as directed, before, during, and after all phases of transitioning. The Specialist will need to observe some variance of work schedules to ensure effectiveness, and assist in gathering data for consistent reporting.
Minimum requirements are an Associate Degree (in social work, behavioral, or other related field) and two years of experience working with individuals with developmental disabilities or other vulnerable populations. Bachelor’ Degree (in social work or related field) and License or Certification in Social Work is highly desired. Must be able to pass pre-employment criminal background and drug testing.
