Guardian Healthcare Providers, Inc. is a trusted nurse staffing partner to facilities which are unique. We seek an Advanced Registered Nurse Practitioner, full-time, for an assignment of at least six weeks, and likely up to 26 weeks, to make a difference at a residential Intermediate Care Facility located in Mexia, TX.

Job Description: Advanced Practice RN

The Nurse Practitioner will be working with electronic health records and software. Must be computer proficient.

The Advanced Practice RN is appointed by the Medical Director subject to the approval of the Facility Director. The Advanced Practice RN is clinically and administratively responsible to the Medical Director. Performs advanced clinical medical care under the supervision of a physician. Work involves assessing, planning, and providing comprehensive patient care independently or in collaboration with health care professionals. Work involves directing medical activities/treatment: monitoring trends and activities, making recommendations for improvement. All other duties as assigned by the Medical Director/Facility Director. May plan, assign, and/or supervise the work of others. Position may possibly serve in an “on call” capacity on a rotating basis as needed. Works under minimal supervision with extensive latitude for the use of initiative and independent judgment. Provide treatment to facility employees whom may be injured in course and scope of employment.

ESSENTIAL JOB FUNCTIONS:
Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
1. Performs acute and chronic care to individuals, to include daily assessment to individuals for acute health problem or injury in the clinic and/or infirmary. The assessment includes diagnosis, treatment and a plan of care with documentation of the acute issue, assessing injuries, including possible sutures and/or referrals to hospital or
emergency room. Chronic care includes a quarterly assessment of identified problems with appropriate treatment specific to the disease process, and monitoring of the chronic condition. Chronic care also includes a quarterly assessment for individuals with identified chronic health care diagnosis including review of appropriate treatment
specific to the disease process and update or change of medical treatment as warranted. 20%

2. Provides comprehensive medical care, which includes conducting rounds, review of infirmary and hospitalized patient and on-call cases for status of condition individuals are under treatment for. Provides medical diagnosis and the review of the treatment plan at the Individual Service Plan (ISP). Includes review of all the identified risks of the individual, (choking, pneumonia, etc.) as well as their health status, and ensures necessary supports (OT, PT, Nursing, etc.) have been identified and appropriate areas of need are being addressed. 60%

3. Develops accurate and timely documentation, which includes dictation of annual summaries and pre-procedural histories and physicals, documentation of specialty medical consultation, physical and chemical restrains and all acute and chronic patient care. 20%

Provide treatment to HHS employees injured in course and scope of employment.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Maintain open and regular communication with the Medical Director and other staff to ensure the transfer of needed information and continuity of services

Provide treatment to HHS employees injured in course and scope of employment.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

LICENSURE, CERTIFICATION or REGISTRATION REQUIRED

Must be licensed as an Advance Practice Nurse by the Texas State Board of Nurse Examiners.
Must have or have the ability to obtain the Drug Enforcement Agency (DEA) and the Texas Department of State Health Services Control Substance Board.
Must possess a National Provider Identifier (NPI#) as defined by Medicare Part B.
